THE HOUSING RELATIONSHIP
Home and care are separate
In supported living, the person has rights in their home and receives support through a separate care or support arrangement. Housing and support responsibilities must be clear before the service begins.
Civic does not present supported living as a care-home placement. Eligibility, tenancy and funding arrangements are discussed with the person and relevant commissioner or representative.

Information we need to consider a referral
Please provide the intended location, age, assessed needs, current risks, communication needs, funding route, desired outcomes and likely support hours. Do not send confidential records through the general enquiry form; we will arrange a secure information route where appropriate.
