We ask who needs support, where they live, what has changed, which tasks may help, preferred times and whether another professional is involved.
We meet the person, listen to their goals and routines, identify risks, discuss communication and consent, and understand the home environment.
The plan states the agreed tasks, timing, outcomes, risks, emergency contacts, recording requirements and how staff will be introduced.
Care begins on an agreed date. Early feedback and regular reviews check whether support is effective and whether the plan needs to change.

FEES AND FUNDING
Care may be funded privately, by a local authority, through NHS Continuing Healthcare or through another agreed route. Eligibility and contribution decisions are made by the relevant funding body. Civic will explain its charges and the agreed service in writing before privately arranged care begins.
